Incident Reporting
It is the policy of the Florida Association of Recovery Residences to require certified providers (or providers in application) to report, within 24 hours, any event resulting in a call or response to or from, 911 at any provider location.
In addition, per Fla Stat §397.487(8)(c), “If any owner, director, or chief financial officer of a certified recovery residence is arrested for or found guilty of or enters a plea of guilty or nolo contendere to, regardless of adjudication, any offense listed in s. 435.04(2) while acting in that capacity, the certified recovery residence shall immediately remove the person from that position and shall notify the credentialing entity within 3 business days after such removal. The credentialing entity shall revoke the certificate of compliance of a recovery residence that fails to meet these requirements.”
Incidents must be reported by the CRRA using the form below.
Upon receipt, FARR staff will acknowledge receipt of the Incident Report and may request further information, if necessary.
*Providers – DO NOT INCLUDE any Protected Health Information (defined as any health information that can identify an individual that is in possession of or transmitted by a "covered entity" or its business associates that relates to a patient's past, present, or future health.)
